Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees, including severe burns, in that employees were exposed to dust explosion, deflagration, or other fire hazards from accumulation of combustible wood dust:        On or about February 7, 2019, and at times prior thereto, in the cabinet shop, the employer did not provide for effective capture and control of combustible dust generated during the operation of woodworking equipment. The dust had been cleaned after an earlier inspection, but was allowed again to accumulate on equipment and building surfaces, exposing employees to the hazards of fire and explosion due to the ignition of the combustible dust.    The Kyllonen Cabinets and Trim LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1), which was contained in OSHA inspection number 1325312, citation number 1, item number 1A and was affirmed as a final order on 1/02/19, with respect to a workplace located at the 702B West Carson St., Bryan, TX.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    On or about February 7, 2019, and at times prior thereto, the employer did not ensure that all  surfaces were kept free of hazardous accumulations of combustible dust.  This exposed employees to the hazard of fire, deflagration and dust explosion in the main shop area.         The Kyllonen Cabinets and Trim LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 1325312, citation number 1, item number 1band was affirmed as a final order on 01/02/19, with respect to a workplace located at 702B West Carson St., Bryan, TX.

29 CFR 1910.307(c): Equipment, wiring and installations of equipment in hazardous (classified) locations was not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    On or about February 7, 2019, and at times prior thereto, non-intrinsically safe equipment such as but limited to electrical panels and receptacles were being used in an area of the facility that was found to be contaminated with combustible dust.      The Kyllonen Cabinets and Trim LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.307(c), which was contained in OSHA inspection number 1325312, citation number 1, item number 5B and was affirmed as a final order on 01/02/19, with respect to a workplace located at 702B West Carson St., Bryan, TX.
