29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a.    On or about February 12, 2019, roofing employees using pneumatic nail guns to install fiberglass shingles were eposed to eye injury hazards from flying debris in that they were not wearing protective eyewear.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.    On or about February 12, 2019, on the 5:12 pitch roof, five roofing employees installing fiberglass shingles were exposed to an 11 feet fall hazard in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.    On or about February 12, 2019, at the eave above the garage, the roofing employees were exposed to a 12 feet fall hazard in that the roof access ladder was not extended at least 3 feet above the edge of the eave.
