On the west side of the church, the front lower wing, employees performing roofing activity on a roof with a pitch of 4 in 12 without fall protection exposing the employees to fall hazards of approximately 25 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On the south side of the church, the front lower wing, employees performing roofing activity were gaining access to an extension ladder that was not properly positioned of having 3 feet above the landing exposing the employees to fall hazards of approximately 20 feet to the ground below.
