29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):     A.)  CJ Michaels Enterprises, Inc. dba Pizza Joes, 604 West Main Street, Grove City, PA  16127, dining area, on or about February 13, 2019 and times prior thereto:  A flexible electrical extension cord was being used to power a Pepsi cooler.  The use of the flexible electrical extension cord was prohibited in that it was being used as a substitute for the fixed wiring of a structure.    B.)  CJ Michaels Enterprises, Inc. dba Pizza Joes, 604 West Main Street, Grove City, PA  16127, counter area, on or about February 13, 2019 and times prior thereto:  A green flexible electrical extension cord was being used to power a telephone.  The use of the flexible electrical extension cord was prohibited in that it was being used as a substitute for the fixed wiring of a structure.    C.)  CJ Michaels Enterprises, Inc. dba Pizza Joes, 604 West Main Street, Grove City, PA  16127, counter area, on or about February 13, 2019 and times prior thereto:  A relocatable power tap was being used to power electrical equipment/devices including, but not limited to, a green flexible electrical extension cord, a cash register and a credit card machine.  The use of the relocatable power tap was prohibited in that it was being used as a substitute for the fixed wiring of a structure.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  A.)  CJ Michaels Enterprises, Inc. dba Pizza Joes, 604 West Main Street, Grove City, PA  16127, counter area, on or about February 13, 2019 and times prior thereto:  The flexible cord for an APW/Wyott WS-4 Heated Shelf Warmer was taunt and the outer jacket of the flexible cord was coming away from the cord cap.  The flexible cord did not have adequate strain relief to prevent the separation of the outer jacket and cord cap.
