29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck.  Bow freezer deck hatch:  On or about 05 February 2019; deflection in roped railing guarding an open hatch measured approximately 29 inches which exposed employees to fall hazards to the lower freezer deck.

29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used:  Bow freezer hold hatch: on or about 05 February 2019; a "mouse and hook" attached to the freezer hold hatch sling was inoperable and not secured correctly to prevent disengagement. This condition exposed employees to struck-by and crushing hazards from falling loads.
