All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. On or about January 30, 2019, the energy control procedure for the thermoforming machine (Brown Machine Co., Model 1424) did not outline the technique to be used to control the pneumatic energy while performing service and maintenance activities, thereby exposing employees to an amputation hazard.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a. On or about February 14, 2019, the thermoforming machine #3421 (Brown Machine Co., Model 1424) had a damaged electrical wire with uninsulated conductor exposed, thereby exposing employees to an electrical shock hazard.
