On or about February 13, 2019, on the second story roof, a roofing employee operating a pneumatic nailer was exposed to and eye injury hazard in that he was not wearing protective eye glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.    On or about February 13, 2019, on the 5:12 pitch roof, employees installing a new roof system were exposed to 8 feet to 18 feet fall hazards from the eaves in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.    On or about February 13, 2019, on the 5:12 pitch roof above the garage, the roof access ladder used by employees to climb from the garage roof to the second-story roof was not extended at least 36 inches above the edge of the upper roof, exposing employees to a 6 feet fall hazard.
