29 CFR 1910.134(e)(2)(ii): The medical evaluation did not obtain the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134: a) Facility: Employees that were provided a 3M half mask respirator were not provided a medical questionnaire that contained information requested from Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134, on or about 1/8/19. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
