Munson Road, Royse City, TX; where employees installing the new roof to a residential single family house were exposed to head injuries where overhead hazard existed by not wearing hard hats.

Munson Road, Royse City, TX; where employees tearing down old roofing tiles and install a new roof to a pre-existing single family house were exposed to fall hazards to the lower level.

Munson Road, Royse City, TX; where employees using extension ladders that did not extend 3-feet above the landing were exposed to fall hazards: (a) Werner 18-feet extension ladder located on the side of the house; (b) Werner 16-feet extension ladder located at the left side at front of the house; (c) Werner 16-feet extension ladder located in the middle at the front of the house; (d) Werner 16-feet extension ladder located at the right side at the front of the house.
