29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) O'Brian Construction at 380 W. Ridges Blvd. Grand Junction, CO 81507: On or before 2/12/19 the creating and exposing employer did not protect employees from injury in that framing work on a new townhome development was performed without the use of approved fall protection systems. Employees were working from ladders and on foot within 6 feet of an unprotected edge. This condition exposed employees to a fall hazard or approximately 12 feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) O'Brian Construction at 380 W. Ridges Blvd. Grand Junction, CO 81507: On or before 2/12/19 the creating and exposing employer did not protect employees from injury in that leading edge work on a new townhome development was performed in part by leaning a step ladder against a post overlooking the leading edge. This condition exposed employees to a fall hazard in excess of 12 feet.
