29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about February 14, 2019, on the 5:12 pitch roof, five roofing employees tearing off old roof shingles were exposed to a nine feet fall hazard in that they were not protected by a fall protection system.

On or about February 14, 2019, at the eave above the garage, the roof access ladder was not extended at least 3 feet above the roof edge, exposing employees to a 9 feet fall hazard.
