Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) 3254 U.S. Route 60, Ona WV: On or about 02/14/19, employees assisting in the spraying of roof coatings on a low slope roof were not protected from falling approximately 21' to the ground below.
