(a) Main Engine Room - North Pump Package: The engine room did not comply with recognized and generally accepted good engineering practices (RAGAGEP), in-that all wall penetrations were not sealed. 1. Pipe runs through the east wall and other east wall penetrations leading from the High Pressure Receiver room into a maintenance area (Tractor Alley) east of the High Pressure Receiver were not sealed to prevent migration of anhydrous ammonia into the room in the event of a release.

PHA Work Sheet L: High Pressure Receiver (PV-102) number two (2) drain valve does not identify safe guard to lock the valve in a closed position. 2. PHA Work Sheet AAA: Ammonia Charging Fill Line does not identify a safe guard to lock the valves in a closed position when a charging operation is not occurring.
