29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about October 24, 2012, and at times prior thereto, employees were allowed to operate machines that were found without adequate guards provided, including but not limited to the following instances:     1) At the press brake machine, a press brake was operated via foot pedal control without guards provided to protect operator and other employees from the point-of-operation.     2) At the shear machine, a power shear had a blade guard provided with an opening of the guard  that would allow employees hands or fingers to enter contact the blade while in operation.     Employees were exposed to amputation and crushing hazards.

29 CFR 1910.217(c)(2): The point of operation guard did not meet the design, construction, application and adjustment requirement:   On or about October 24, 2012, and at times prior thereto, at the Bliss #3 and Niagara mechanical power press, the guards in the back of the machines had parts of the guard missing or not secured so that they could not be opened  easily by hand without using any special tool.   Employees were exposed to  amputation and crushing hazards.
