29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  West side of the residential construction site: On or about February 15, 2019, and at times prior thereto, employees were exposed to a trip and fall hazard while accessing the roof with debris, pallets and metal panels on the floor.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  West side of the residential construction site: On or about February 15, 2019, and at times prior thereto, employees were exposed to struck-by flying projectile hazards, while using nail guns for installing metal panels, without safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  West side of the residential construction site: On or about February 15, 2019, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the unpaved ground below while installing metal panels on the roof.
