Located at 60 E Spring St, Columbus, OH: On or about February 15, 2019, the employees used a 10 foot Werner fiberglass ladder to access the roof top that was approximately 13 feet high. The employees used the ladder above the maximum working height of 10 feet, thereby exposing employees to a fall hazard of 10 feet.
