29 CFR 1904.7(b)(3)(ix): The employer failed to estimate the total number of calendar days for an injury or illness that happened in the previous calendar year on the OSHA 300 log. This resulted in the employer's annual summary to be inaccurate. a) Care Plus Bergen, Inc. DBA Bergen New Bridge Medical Center: The employer did not enter correctly the work-related injuries in which a mental health assistant suffered the fracture of 3 nasal bones as a result of workplace violence. On the OSHA Form 300 or equivalent for the calendar year 2019 the employer entered 46 in column K, but did not update the total days away to 73 when the number of days away became known, on or about February 19, 2019.
