29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C) were dispensed into containers without electrically interconnecting the nozzle and the container:    On or about February 15, 2019, employees were exposed to fire and smoke inhalation hazards when they dispensed PPG and Sherwin Williams coatings mixed with Vietek Wash Thinner without electrically interconnecting a funnel used to catch the spent solvent wash from the paint guns into a 55-gallon metal drum.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about February 15, 2019, employees were exposed to eye injury (corrosive coatings and battery acid) and the employer provided a Honeywell Porta Stream emergency eyewash (Model 2300; S/N 540232) that contained stored water which was unsuitable for use because it was not changed as required by the manufacturer.

29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s):    On or about February 18, 2019, employees operated a Yale Model ERC060HBN365V083 (Serial Number N448267) forklift and the employer failed to ensure all powered industrial vehicles were removed from service when found to be in need of repair due to the following:    a) brake failure; and     b) exposed inner wiring at the battery terminal.

In February 2019, the employer did not complete daily inspections for brake function and employees in the Fab Weld Department were exposed to struck-by hazards when they used the 20-ton crane (Make MMM; Model 2; S/N 56-36355BM) and the bridge brake did not work properly.
