29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a.) Blast Area:  An employee sand blasting metal material was exposed to 130% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 91.8 dBA during the 180 minute sampling period on February 28, 2019. The exposure calculation included a zero increment for the three hundred minutes not sampled. The employer has not established a hearing conservation program.

Blast Area:  An employee sand blasting metal material was exposed to 130% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 91.8 dBA during the 180 minute sampling period on February 28, 2019. The exposure calculation included a zero increment for the three hundred minutes not sampled. The employer has not established a monitoring program.

Blast Area:  An employee sand blasting metal material was exposed to 130% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 91.8 dBA during the 180 minute sampling period on February 28, 2019. The exposure calculation included a zero increment for the three hundred minutes not sampled. The employer has not provided the employees with annual audiograms.

The exposure level was derived from a sample collected over a 60 minute sampling period on 2/28/19. Zero exposure assumed for the 420 minutes not sampled.
