Energy Control Procedures were not developed and documented for authorized personnel to follow when accomplishing maintenance and servicing work on the powered industrial vehicles exposing employees to accidental/inadvertent startup or hazardous movement of the equipment.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:     On or about February 13, 2019, employee(s) were exposed to machine hazards associated with crushing when they engaged in die change/set-up activities on the Bystronic Beyler PR8 150. The machine was not shut down or turned off prior to engaging in this servicing activity.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards."
