29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  On or about February 15, 2019 at Hwy 138 Stockbridge , GA 30281, the employer exposed employees to hazards associated with employees operating a rough terrain forklift without training.  Employees used a CAD Model: TL1055D SN: ML501428 to lift materials to the upper floors of the structure.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about February 15, 2019 at Hwy 138 Stockbridge , GA 30281, the employer did not ensure that all areas were employees are working where free of debris including, but not limited to plywood and cardboard boxes exposing employees to trips and falls.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or perso(a):  On or about February 15, 2019 at Hwy 138 Stockbridge , GA 30281, the employer did not ensure that unprotected edged on the stairs were guarded when  were employees ere exposed to falls of approximately 8 feet to the dirt below when accessing the upper floors.
