29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    On or about 2/5/2019 throughout the facility where the employer failed to provide authorized training to operators performing authorized activities requiring lockout, including but not limited to troubleshooting and cleaning on equipment  including but not limited to the LT8 line. Operators and maintenance personnel made full body entrance multiple times daily relying on a key that was not removed and maintained on their person, an E-stop, and interlocked door.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    On or about 2/5/2019 throughout the facility employees were exposed to amputation, caught in, and struck by hazards where the interlocks, E-stop, and turning power off at key were relied upon for protection on equipment including but not limited to the LT8 line while performing tasks including but not limited to troubleshooting and cleaning. Operators made full body entrance multiple times daily without the application of lockout device(s) and relying on turning the power off at the key, leaving the key in place, hitting the E-stop, and opening the interlocked door.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about 2/5/2019 throughout the facility on equipment including but not limited to the LT8, workers performing operations including but not limited to cleaning and troubleshooting are exposed to amputation and struck by hazards where the employer failed to identify in writing:    a. the purpose of the written specific procedures;  b. types and magnitude of energy sources; and   c. verification of control of energy sources.    Joseph T. Ryerson & Son, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1347972, citation number 1, item number 1 and was affirmed as a final order on 11/13/2018, with respect to a workplace located at 4405 South Old Peachtree Road Norcross, GA 30071.
