29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about Febuary 14, 2019, on the 17th floor, maintenance yard area, of the building located at 1999 West County Club Drive, Aventura, FL, an employee was installing wire for lightning protection from the top of a wall without the employer enforcing the use of the fall protection equipment that was provided by the employer, thus exposing him to a 20-foot fall.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: On or about February 14, 2019, on the 17th floor, maintenance yard area, of the building located at 1999 West Country Club Drive, Aventura, FL, an employee was installing wire for lightning protection from the top and tied to a green rope that was attached to PVC pipe, exposing him to a fall of 20 feet.
