29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about February 6, 2019, in the Forging Department, the employer did not ensure that employees working near unprotected openings along the sides of the six (6) inch Upsetter Machine No. 837, SN #3773, were protected by the use of an effective guardrail system, thereby exposing the employees to fall hazards greater than four (4) feet into the adjacent pit.    b)  On or about February 15, 2019, in the Forging Department, the employer did not ensure that employees working near unprotected openings along the sides of the eight (8) inch Upsetter Machine No. 836 were protected by the use of an effective guardrail system, thereby exposing the employees to fall hazards greater than four (4) feet into the adjacent pit.    c)  On or about February 15, 2019, in the Forging Department, the employer did not ensure that employees working near unprotected openings along the sides of the five (5) inch Upsetter Machine No. 1041, SN #1664, were protected by the use of an effective guardrail system, thereby exposing the employees to fall hazards greater than four (4) feet into the adjacent pit.
