29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 02/19/19, on a roofing jobsite at a house at 181 West Ave., Lockport, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 20 foot. Lifelines anchored to the roof were present on the roof. One of the employees was wearing a full body harness and lanyard, but was not tethered to a lifeline. The second employee was a humper, and was not wearing a harness. Pitch of the roof was approx. 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 02/19/19, on a roofing jobsite at a house on 181 West Ave., Lockport, NY: Employees used an extension ladder to access the roof on the right front side of the house. The siderails of that extension ladder extended approx. 30 inches above the edge of the roof. NO ABATEMENT CERTIFICATION REQUIRED
