WORKSHOP:  The employer had not secured the brick and wooden steps (Steps) placed under the foot pedal of the table top wire bender, unknown identification.       On or about October 3, 2018, the Steps were displaced when the operator tripped the foot pedal while the operator's right point finger was still in the wire bender, and the wire bender caught the right point finger resulting in finger tip crushing injury and later in November 2018-January 2019 resulted in a finger tip amputation.
