Procedures did not specifically outline the scope, purpose, rules and techniques to be utilized for the control of all hazardous energy associated with the Omega-American Hypro II ironing machine. Employees were exposed to unexpected energization or startup of the machines or equipment, or release of stored energy causing crushed-by, caught-between, struck-by or electrical shock hazards.
