29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: (a) This violation occurred on or about February 20, 2019, at the job site located at 300 Trail House Lane, Forney, TX; employees were working cutting plywood with a Dewalt circular saw. Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) This violation occurred on or about February 20, 2019, at the job site located at 300 Trail House Lane, Forney, TX; where employees were decking a new residential apartment building #3 and were exposed fall hazards to the lower level.
