29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a) On or before February 20, 2019, employees at the site were exposed to fall hazards when a ladder was not installed for accessing the multi-leveled scaffold platform.

(a)    On or before February 20, 2019, employees at the site were exposed to fall hazards greater than 10 ft. when no form of fall protection was used while working from the 2nd level of the multi-leveled scaffold platform.   The employer, RM Masonry and Stucco, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1366088, citation number 1, item number 4 and was affirmed as a final order on 2/11/2019, with respect to a workplace located at 705 W. Avenue B, Garland, TX.
