On or about February 13, 2019, employees were operating the #942 Tube Mill Cut Off Machine located in the Mill Department, and the side access door on the machine was not properly guarded to protect employees from hazards created by point of operation, thereby, exposing employees to amputation hazards.    b. On or about February 22, 2019, employees were operating the #942 Tube Mill Cut-Off Machine and the rotating shaft in the sizing area was not guarded, thereby, exposing employees to amputation hazards.    Metal-Matic, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1165982, citation number 1, item number 3 and was affirmed as a final order on January 31, 2017, with respect to a workplace located at 1701 Made Industrial Drive, Middletown, OH 45044.
