29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: (a) Employees working on the P97 Cincinnati Milacron injection molder located in Department 9 were exposed to fall hazards of approximately 49 inches off the machine side of the unguarded open sided wooden platform. (b) Employees working on the P27 Cincinnati Milacron injection molder located in Department 9 were exposed to fall hazards of approximately 54 inches off the machine side of the unguarded open sided wooden platform. (c) Employees working on the mezzanine located above the plastic injection molders in Department 9 were exposed to fall hazards of approximately 12 feet in-between guardrail openings that were guarded with a single chain.

(b) Employees working on the P27 Cincinnati Milacron injection molder located in Department 9 were exposed to fall hazards on stairs leading to a wooden platform that were not provided stair rail systems and handrails on both sides.
