On or about February 21, 2019, Employees performing roofing activities were exposed to fall hazards on a residential construction site in Champaign, Illinois, and the employer did not ensure an adequate fall protection system was being used. JESUS ORTIZ DBA CBI CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1365509, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 29, 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 408 DENTON DRIVE, CHAMPAIGN, ILLINOIS, 61874.
