On or about 2/22/2019 at 4026 N Expressway, Hampton, GA: The employer failed to report to OSHA a work-related hospitalization and amputation within 24 hours of such knowledge and did not contact OSHA of the following injury: On or about 2/16/2019, Erik Rafter a Labor Worker was hospitalized with leg injuries. OSHA became aware of this employees hospitalized injuries on 2/20/2019.

On or about 2/22/2019, at 4026 N Expressway, Hampton, GA: Employees operating the propane powered industrial truck to lift and move equipment and 55 gallon drums of water were not provided complete and all practical training and evaluation of their performance prior to operating the powered industrial truck.
