29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. On or before February 20, 2019, in the press area, the employer failed to ensure that their specific lockout/tagout procedure for the Bliss C-150 power press number 499 (S/N: H65023) identified stored energy sources and had specific procedural steps to address stored or residual energy and gravity sources including the following: relieving/bleeding off stored air pressure between the storage tank and check valve at the manual bleeder valve and placing a blocking or similar device under the die head. On or about February 11, 2019, an employee sustained a fingertip amputation while attempting to remove the counter balance (cylinders) for a rebuild on the Bliss C-150 power press number 499 due to the stored air pressure not being relieved/bled off.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about February 11, 2019, in the press area, the employer failed to ensure that employees who were removing the counter balance (cylinders) for a rebuild on the Bliss C-150 power press number 499 (S/N: H65023) had the knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; perform an orderly shutdown by completely shutting down press number 499; isolate the press number 499 from the energy sources; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy for press number 499; and verify that isolation and deenergization of press number 499 had been accomplished, thereby exposing employees to a struck-by and caught-in hazard. On or about February 11, 2019, an employee sustained a fingertip amputation while performing the aforementioned activity due to the stored air pressure not being relieved/bled off between the storage tank and check valve.
