29 CFR 1910.1200(e)(1)(i): The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate safety data sheet:    (a)Lake Norden Care and Rehabilitation, LLC at 803 Park Street, Lake Norden, SD 57248: On or about and at times prior to February 22, 2019 the employer did not ensure that the company's hazard communication program included a list of the hazardous chemicals known to be present in the workplace. This condition exposed employees to chemical hazards.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged, or marked with the information specified under paragraphs (f)(1)(I) through 29 CFR 1910.1200(f)(1)(v):  (a)Lake Norden Care and Rehabilitation, LLC at 803 Park Street, Lake Norden, SD 57248: On or about and at times prior to February 22, 2019, the employer did not ensure employees were protected from chemical hazards in that, a spray bottle of EcoLab A-456 II Disinfectant Cleaner was not labeled, tagged, or marked with the information specified under paragraphs (f)(1)(I) through 29 CFR 1910.1200(f)(1)(v). This condition exposed employees to chemical hazards.
