29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a) On or about February 22, 2019; south-side (rear) of roof; employees were not wearing eye protection while using equipment such as but not limited to hammers to remove the shingles from the roof.      No abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about February 22, 2019, south-side (rear) of residential build; employees were observed removing the roof of a residential home without fall protection in place to protect them falling approximately 20-25 feet to ground level.       No abatement certification or documentation required for this item.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about February 22, 2019; the employer had not certified the fall protection training had been provided to employees that were exposed to fall hazards 6-feet or greater while performing roofing work.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet.
