On or about February 13, 2019 at the pipe yard where employees were exposed to struck-by hazards while operating a John Deere 744J forklift SN: DW644JX612417 that was being operated with low tire pressure.      2. On or about February 21, 2019 at the pipe yard where employees were exposed to struck-by hazards while operating a John Deere 644H forklift SN: DW644HX573717 that had a horn that did not work.
