29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): a) On or about March 20, 2019 on the #2 roll line: The employer failed to ensure group lockout was used when an employee assisting with replacing the base plate on the #2 roll machine properly locked out the machine according to the companies specific lockout procedures, in that only the e-stop was used to control the electrical and pneumatic energy sources, which exposed the employee to amputation and other injuries related to the inadvertent startup of equipment. b) On or about March 20, 2019 on the #2 roll line: The employer failed to ensure group lockout was used when an employee removing and replacing the base plate on the #2 roll machine properly locked out the machine according to the companies specific lockout procedures, in that only the e-stop was used to control the electrical and pneumatic energy sources, which exposed the employee to amputation and other injuries related to the inadvertent startup of equipment.
