29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval. a. On or about February 22, 2019, at the above addressed jobsite, employees were lifting an unapproved job made box on the forks of a Gehl RS10-55 Rough Terrain Forklift. This practice could affect the capacity and safe operation of the equipment and the manufacturers written approval was not obtained, thereby exposing the employees to a struck by hazards.
