29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)    An employee was exposed to a corrosive chemical (DuBois IWT-4B) which is pumped into a 5 gallon pail and then dumped into a hopper or the Water Jet Machine, 20-01 in Bay X and was not provided with suitable facilities for flushing eyes.  The small eye wash bottles on the wall near the machine would not provide for continuous eye washing for 10 minutes as described on the SDS or provide for washing contaminated clothing.

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:    a)    On or about 2/28/2019, the Red forklift, had a several deficiencies and was not removed from service.  The seat belt slide was broken that stops seat belt from hooking, one of the forks would not lock in place to secure the load, foot pedals did not have the rubber pads to keep feet from slipping off of them, there was a damaged/missing lens cover for light behind seat, and the ID tag for lift truck with weight and attachment description was not legible.    b)    On or about 2/28/2019, the Standup lift truck raising cage lever was no longer operable.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)    On or about 3/5/2019, there was an ingoing nip point at bottom of sander that was located in the Die Shop.    b)    On or about 3/5/2010, the lathe in the Die Shop was not equipped with a chuck guard.    c)    On or about 3/5/2019, there was a nip point hazard created by the chain conveyor on parts washer 890-01 in W Building.    d)    On or about 3/5/2019, the horizontal band saw was missing the guard for the unused portion of the blade.
