29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about 2/22/2019, energy control procedures for equipment including, but not limited to, the horizontal band saw was not utilized for the performance of activities such as changing the blade, in that, a lockout device was not applied, exposing employees to an amputation hazard. The Proto, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1042785, citation number 02, item number 001 and was affirmed as a final order on 6/3/2015, with respect to a workplace located at 10500 47th Street N, Clearwater, FL 33762.

(b) On or about 2/22/2019, the blade of the horizontal band saws was inadequately guarded, exposing employees to an amputation hazard. The Proto, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1042785, citation number 01, item number 002 and was affirmed as a final order on 6/3/2015, with respect to a workplace located at 10500 47th Street N, Clearwater, FL 33762.
