29 CFR 1910.217(c)(3)(iv)(d): Each pull-out device in use shall be visually inspected and checked for proper adjustment at the start of each operator shift, following a new die set-up, and when operators are changed. Necessary maintenance or repair or both shall be performed and completed before the press is operated. Records of inspections and maintenance shall be kept in accordance with paragraph (e) of this section.        a) Inspection records for the Pull back devices for the Bliss Presses were maintained to ensure proper distance from pinch point hazards, in that operators were inspecting/checking themselves for proper distance.

Flywheels located so that any part is seven (7) feet or less above floor or platform shall be guarded in accordance with the requirements of this subparagraph:      a)  The guarding for the Flywheel on the Bliss Screen Press was inadequate in that the wooden guard had an opening in it exposing the operator(s) to struck by hazards.

This subparagraph does not apply to keys or setscrews within gear or sprocket casings or other enclosures, nor to keys, setscrews, or oil cups in hubs of pulleys less than twenty (20) inches in diameter where they are within the plane of the rim of the pulley.       a) The shaft for the Nail machine was not guarded exposing employees to rotating parts.    b) The shaft for the conveyer line attached to the Bliss press was not guarded exposing employees to rotating parts.
