29 CFR 1910.410(a)(2)(i): Each dive team member did not have experience or training in the use of tools, equipment and systems relevant to assigned tasks: a) Pier 14-N, Naval Station Norfolk: On or about 01/25/2019, DOD civilians required to wear an Air III Navy Dive Computer during a surface supplied air dive were not properly trained to use the computer. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.421(d)(6): Planning of a diving operation did not include an assessment of the safety and health aspects of the dive team assignments and physical fitness of dive team members (including any impairment known to the employer): a) Pier 14-N, Naval Station Norfolk: On or about 01/25/2019, a visually impaired diver was not physical fit to dive using a Navy Air III Dive Computer in that the diver had difficulty seeing the display resulting in another diver reading the display for him. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.421(d)(8): Planning of a diving operation did not include an assessment of the safety and health aspects of the decompression and treatment procedures (including altitude corrections): a) Pier 14-N, Naval Station Norfolk: On or about 01/25/2019, a DOD civilian diver while diving a no-decompression surface supplied air dive was not monitored by Tables 9-7 and 9-8 per the U.S. Navy Dive Manual. Note: No-decompression limits: The depth-time limits of the "no-decompression limits and repetitive dive group designation table for no-decompression air dives", U.S. Navy Diving Manual or equivalent limits which the employer can demonstrate to be equally effective. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
