29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 02/20/2019, on mezzanine area, employee went on conveyor #7 that was approximately 10½ feet above the floor to cross over to conveyor #6 to fix boxes that had jammed on the turn. Employee subsequently fell to the ground due to the conveyor belts having unprotected sides and no alternative means of fall protection.  b) On or about 2/26/2019, on the mezzanine area, employees are exposed to falls when performing regular job duties that require them to get on the conveyor belt with guardrails that consisted of only the top rail (approximately 10½ feet) such as, but not limited to, changing the truckload numbers on clipboards that are in front of the scanners above conveyor, or moving milk crates past the conveyor belt to rollers (approximately 8 feet from mezzanine). Employees are not given alternative means of fall protection.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) On or about 02/26/2019, in the Mezzanine Area, employees did not utilize lockout procedures when doing maintenance to the conveyor belts, such as but not limited to, replacing electrical motor, exposing maintenance employees to amputation hazards or fall hazards if belt is energized while employees are standing on the belt.   b) On or about 02/26/2019, in the Mezzanine Area, employees did not utilize lockout procedures when unjamming conveyor belts exposing employees to amputation hazards or fall hazards if belt is energized while employees are standing on the belt.   c) On or about 02/26/2019, throughout establishment, lockout procedures were not developed for equipment with multiple sources of energy such as, but not limited to, two sources of electrical energy need to be locked out when performing maintenance on the motor on the conveyor belts.
