29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about 02/25/2019, Location: 10286 NW 9th Street Circle, Indian Lake Village, Miami FL, 1) Employees were exposed to fall hazards while engaged in roofing activities replacing shingles on a flat surface approximately 20ft above lower level and not protected from falls by personal fall arrest system or a combination of warning line system and safety monitoring system. Employees were wearing harnesses, but not secured to any lifeline or anchoring point. Warning lines were not in place and safety monitoring was not being performed.
