29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a. On or about February 25, 2019, the employees installing shingles on the roof of the building located at 14 Palm Harbor Village Way, Palm Coast Florida, using a Hitachi NV 45AB2 pneumatic coil-nailer were not protected from flying particles by the use of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about February 25, 2019,  four employees installing shingles on a roof with a 5:12 pitch, of the building located at 14 Palm Harbor Village Way, Palm Coast Florida, were not protected from falling more than 11 feet to the ground by the use of a fall protection system.         Flagler County Roofing, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1183668, citation number 1, item number 1 and was affirmed as a final order on 11-29-2016, with respect to a workplace located at 6 Maple Street, Flaggler Beach, Florida.
