On or about February 26, 2019 and at times prior thereto, the employer did not ensure Permit Required Confined Space Entrants wore a retrieval system while in a confined space, exposing employees to the hazards of a confined space. NOTE: Wristlets may be used in lieu of the chest or full body harness if the employer can demonstrate that the use of a chest or full body harness is infeasible or creates a greater hazard and that the use of wristlets is the safest and most effective alternative.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. On or about February 26, 2019 and at times prior thereto, emergency eyewash stations within work areas for employees exposed to corrosive materials such as but not limited to, Ferric Sulfate Solution, were not maintained in a working condition, exposing employees to the hazard of corrosive chemical splash: a) Eyewash station between tank ST1 and ST2 were missing a spray head and plumbing did not operate. b) Eyewash station near ventilation hood were missing spray heads, steel receptor was broken from drainage pipe and plumbing did not operate.
