29 CFR 1926.404(f)(6): The path to ground from circuits and equipment was not permanent and continuous: a) Employees were exposed to 110v electric shock hazard when using concrete vibrator machine powered by a blue extension cord with broken ground pin. Location: 120 Java Street, Brooklyn NY On or about 2/22/2019 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
