29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Machine Shop Building located at 5555 N. 119th St. W. Maize, KS 67101: Employee engaged in preventive maintenance activities, and other affected employees were exposed to electrical, amputation, caught-in, and struck-by hazards associated with the unexpected energization or start-up of equipment such as but not limited to the SNK, the Haas, the UMC, and the Fidia CNC machines. Employer did not assess all equipment and establish a control of hazardous energy written program consisting of energy control procedures, and employee training to identify potential hazards of unexpected energization or start-up or release of stored energy including electrical, hydraulic, pneumatic and gravity. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Machine Shop Building located at 5555 N. 119th St. W. Maize, KS 67101: Employee engaged in servicing and maintenance activities was exposed to electrical, amputation, caught-in, and struck-by hazards associated with the unexpected energization or start-up of equipment including but not limited to the SNK, the Haas, the UMC, and the Fidia CNC machines in that machine specific energy control procedures had not been developed, documented, and utilized. Sources of hazard energy include electrical, hydraulic, pneumatic and gravity. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
