On or about February 27, 2019, located at Lot 66 6813 Brooklyn Heights Rd, Westerville, OH 43081, employees were performing excavation work and installing three and six inch sewer line without any means of protection from cave-ins by an adequate protective system, the employer failed to maintain an effective safety and health program that protected employees from hazards while performing excavation work.

On or about February 27, 2019, located at Lot 66 6813 Brooklyn Heights Rd, Westerville, OH 43081, employees were performing excavation work and installing three and six inch sewer line without any means of protection from cave-ins by an adequate protective system, the employer failed to ensure cave-in protection was provided for employees working in an open trench, approximately seven to ten foot in depth.
